What is My Aged Care?

My Aged Care help you access aged care services and information in Australia.

My Aged Care is the first point of contact to find out about services that can support you, if you are 65 years and over, or 50 years for Aboriginal and Torres Strait Islander people.

How do I make a referral?

From the 1 August 2016, referrals to access a Merri Health aged care service, will need to go through My Aged Care. Call My Aged Care and tell them you want Merri Health as your preferred provider. They will then arrange an assessment for you.

What services are affected by My Aged Care?

Call My Aged Care to refer to the following services:

Health services

Wellness at Home - Home care packages

Support to help frail older people who have complex or changing care needs, to stay living in their home and community for longer. This service is known as home care packages.

Social groups

Social groups were known as centre-based planned activity groups.

Northern respite services
