Support for people with chronic conditions

Supporting you to develop healthy habits to prevent or better manage diabetes, heart or lung disease.
What is it?
Be Well, Live Well is an 8 week program suitable for people who have, or are at risk of, chronic cardiac or respiratory disease or diabetes, or people who need to change their lifestyle.
The program provides group and individual education and exercise that supports you to:
- Stay as healthy and well as possible
- Build confidence and knowledge to make healthier choices
- Feel better again after being unwell or in hospital
Carers and support people are welcome.
Sessions are currently being conducted online via telehealth with some onsite exceptions.
Who can use this service?
Anyone 18 years and over that has, or are at risk of, chronic cardiac or respiratory disease or diabetes, or people who need to change their lifestyle.
Cost
It's free for telehealth sessions. Charges may apply with group settings. Please contact us for more information.
Do I need a referral?
No, you do not need a referral to access this service. You can self-refer or you may have a referral from a doctor, health professional or hospital.
Location and time
Mondays, 2-4pm at Wurundjeri Woi Wurrung | 93 Bell Street, Coburg or via telehealth.

No.
Will I need a pre-assessment?
No, an initial assessment is completed with our clinicians before starting the program.
