Your new role

This role is the chance to join a passionate and highly skilled team multidisciplinary team working to support people with dementia and their carers. In this role, you will work with clients to provide support, information and access to services with a view to improving or maintaining their quality of life. Your role will foster a wellness approach, which focuses on clients’ strengths, is goal-directed and aims to maximise independence and autonomy.

There is also an opportunity to further develop the social worker role within the team. You will be encouraged to innovate and share ideas through involvement in quality improvement initiatives within the organisation, and contribute to ongoing service development initiatives including research and evaluation activities.

You will be responsible for

You will support people living with dementia and their carers through providing one-on-one to education and information to develop their understanding of dementia. You will also assist clients to access services and support which will help to maintain their quality of life. In addition, the role involves facilitating groups for carers of people living with dementia, with a focus on peer support, stress reduction and skill development.

As part of this role you will also work with vulnerable older persons to provide linkages support for social inclusion, mental wellbeing and physical health through comprehensive assessment, emotional support, service coordination, and referral.

To succeed in this role you will have

- Bachelor Qualification in Social Work.
- Knowledge of the community care sector, including the challenges faced by faced by older people, carers and those with chronic illness or disability.
- An interest in working with people with dementia and their carers
- Excellent interpersonal skills
- An ability to work as part of a multi-disciplinary team
