Meet the physician team based at The Royal Melbourne Hospital and the community health teams from the Hospital Admission Risk Program (HARP) and Merri Health.

This session will provide an update from respiratory experts at the acute and primary care interface. Learn evidence based management of chronic respiratory disease and explore current treatment options from diagnosis to management of advanced lung disease, including community and hospital care.

On completion of this course, attendees will be able to:

• Utilise session details and specialist information to identify referral pathways and options to ensure integrated, person-centred care.
• Describe the appropriate tests to diagnose various forms of lung disease.
• Develop a comprehensive management plan for patients with respiratory disease from diagnosis through to end of life.
