Introduction and referral pathway into Merri Health’s new Chronic Pain Service
Do you you have patients living with persistent pain? Are they ready to engage in an active management, multidisciplinary approach to their pain care?
Our community health based Chronic Pain Service provides biopsychosocial pain care through care coordination, pain physiotherapy, clinical psychology and medical support.
This service is a 2 year PNH funded collaboration with The Royal Melbourne Hospital, The Northern Hospital and General Practitioners
The service supports people aged 18 and over who have persistent pain and live in Hume, Darebin, Moreland, Yarra, Melbourne, Moonee Valley or the Macedon Ranges area.
