What is Well @ Home?
Well @ Home provides fast access to allied health and nursing services for people with a HCP providing more clinical care and choice.
Our dedicated intake and admin support team work with Case Managers in a fast and easy process. We will ensure services are delivered on time and within budget.
Who can use this service?
Clients living in Northern Metropolitan Melbourne who have been assessed and approved for a Home Care Package. Case Managers or clients themselves can contact Merri Health to initiate services.
