Your new role

The Team Leader - Prevention and Chronic Illness Care Team will be responsible for:
• Providing leadership and direction to the multidisciplinary staff in day to day operations of the team.
• Support the team to deliver high quality, evidence based services to the community and in line with Merri Health’s strategic directions.
• Support the implementation of innovative, appropriate, effective and efficient models of care.

You will be responsible for

Provide leadership within the team to ensure the provision of safe and evidence-based services and multidisciplinary practice.
Ensure the services provided by staff are consistent with policies and practices that align with professional clinical standards.
In collaboration with other PCIC Team Leaders, convene and lead team meetings monthly.
In Consultation with the manager be responsible for team outcomes, ensuring targets and funding requirements are met.

• Tertiary qualifications in Allied Health, Nursing or Community Health Sector disciplines
• Post graduate qualifications in a relevant field or equivalent experience
• Demonstrated ability in leading a multidisciplinary team
• Demonstrated understanding of chronic disease frameworks and the use of best practice guidelines around prevention, risk factor modification and chronic disease management.
